Health Assessments
We offer the service of a comprehensive annual health review for patients aged over 75 years of age (or 55 years for Aboriginal and Torres Strait Islander patients). The Health Assessment is an opportunity to explore areas of health that may not be covered during routine medical care. Areas covered in the assessment are:
- Physical ability (how well you manage daily tasks)
- Social Support (what support you have)
- Psychological well being (how you cope)
These assessments are done in two parts. Firstly the practice nurse will perform an interview with you, either at your home (preferably) or at the surgery which will take one hour. You will then attend your normal doctor for a medical check up and to complete the holistic assessment.
When making an appointment, please advise the receptionist that you wish to make an appointment with the health assessment nurse to come to your home.
45-49 YEAR OLD MEDICAL CHECKS
As part of our preventative medicine program, we can offer a check up for 45-49 year old men and women. This is designed to detect and prevent chronic disease for people with high risk factors, including high blood pressure, high cholesterol, diabetes, family history of chronic disease, smoking, physical inactivity, nutritional deficiencies, alcohol or drug abuse and excess weight.
Your appointment for this medical check takes only 20 minutes, and when making an appointment you should tell the receptionist the appointment is for a 45-49 year old medical check.
