Medication reviews

If you take multiple medications or have ongoing health conditions, a medication review can help make sure your medicines are still safe, effective, and working well together.

During a medication review, your GP works with an accredited pharmacist to:

• review your current medications and supplements
• check for side effects or interactions
• simplify medication schedules where possible
• improve safety and effectiveness
• answer any questions about your treatment

Medication reviews are especially helpful for people taking several medications, those recently discharged from hospital, or anyone experiencing side effects or confusion about their medicines.

Your GP can let you know if you are eligible and arrange the referral.
