Health Assessments

We offer annual, comprehensive health assessments performed by both our experienced nurses and GPs.

These assessments provide a holistic review of your overall wellbeing—an opportunity to explore aspects of your health that may not be covered during routine appointments.

Health assessments are available to a wide range of eligible groups, including:

  • People aged 75 and over
  • People aged 40-49 at high risk of developing diabetes
  • People aged 45-49 at risk of chronic disease
  • Aboriginal and Torres Strait Islander health assessment
  • People with an intellectual disability
  • Refugees and humanitarian entrants

Health Assessments are done in two parts:
Firstly, a practice nurse will perform a comprehensive interview, either at the surgery or in your home, which takes about an hour:
Subsequently you attend your regular doctor for a medical checkup and completion of the assessment.

GP’s are happy to bulk bill the Over 75 Health Assessment so there is no cost to you. Speak to your doctor or one of the nurses if you would like a Health Assessment.
